By accessing or using the services of Gravitate Wellspace, you acknowledge and agree to the following Terms & Conditions. These terms govern all treatments, appointments, and interactions with Gravitate Wellspace and its practitioners.
Gravitate Wellspace provides chiropractic care and wellness services intended to support musculoskeletal health and overall well-being. While chiropractic care is widely recognized as a safe and effective form of treatment, all services are provided “as is,” with no express or implied guarantees of specific results.
Gravitate Wellspace requires a valid credit card on file to book any appointment. This card may be charged in accordance with our cancellation and no-show policy.
Gravitate Wellspace does not offer refunds, returns, exchanges, or credits under any circumstances. This policy applies to all services, and any future physical products that may be introduced. All payments are final, and services are non-transferable. Additionally, once a product has left our facility, it cannot be returned, exchanged, or refunded for any reason.
Appointments must be scheduled in advance, and clients are responsible for providing accurate health history before treatment. Failure to disclose relevant medical information may impact the effectiveness and safety of care. A minimum of 24 hours' notice is required for cancellations or rescheduling.
Failure to provide at least 24 hours' notice for cancellations or rescheduling will result in a cancellation fee charged to the credit card on file. No-shows will also be charged a cancellation fee. Additionally, late cancellations or no-shows may result in forfeiture of the appointment. Gravitate Wellspace reserves the right to restrict or block future bookings for individuals who repeatedly fail to comply with appointment policies.
By receiving treatment at Gravitate Wellspace, clients acknowledge and assume all risks associated with chiropractic care, including but not limited to temporary soreness, discomfort, or adverse reactions. Gravitate Wellspace and its practitioners are not liable for any direct, indirect, incidental, special, or consequential damages arising from the use of its services, including but not limited to personal injury, loss of function, or dissatisfaction with treatment outcomes. Clients release Gravitate Wellspace, its chiropractors, employees, and affiliates from any liability related to injury, reaction, or undesired outcome resulting from treatment.
All client records and health information are kept confidential and used solely for treatment purposes. Gravitate Wellspace complies with all applicable privacy regulations, including HIPAA, to protect client information.
Gravitate Wellspace reserves the right to refuse service, restrict website access, or block individuals from booking appointments at its sole discretion. Reasons for refusal may include, but are not limited to, inappropriate behavior, failure to comply with policies, failure to disclose relevant health information, chargebacks or payment disputes, or any condition that, in the chiropractor’s professional judgment, makes treatment unsafe or inappropriate.
Gravitate Wellspace reserves the right, at its sole discretion, to modify or update these Terms & Conditions at any time without prior notice. The most current version will supersede all previous versions, and continued use of services constitutes acceptance of any updates.
These Terms & Conditions are governed by the laws of the State of Florida. Any disputes arising from the use of services shall be resolved exclusively in the courts of Florida. If any provision of these Terms & Conditions is deemed invalid or unenforceable, the remaining provisions will remain in full force and effect.
By scheduling an appointment and receiving care at Gravitate Wellspace, you acknowledge that you have read, understood, and agree to these Terms & Conditions.
